Attach files to entities

Enables users to instantly upload almost any file format to Prescience.
Documents are to be stored in relation with the entity, making it easy for users to see which documents pertain to which entities.

It should be possible to add some meta data to the attachment, in addition to the mandatory time, user and file format info, such as:

• File Type (quality document, certificate, etc)
• File description
• more?
